PARENTS

2015-2016 Before and After School Needs Survey

In order to determine if a before and after school program will be offered at your school, we are asking parents of children currently in Kindergarten to Grade 5 to fill out the short survey below. Your responses will help us determine if a program in your school will meet or continue to meet minimum enrolment, as well as the hours of care required for families in your school community.
The survey is open until February 16, 2015: http://fluidsurveys.com/s/scdsbbeforeandafterchildcare2015/
Once decisions about where B&A programs will operate in the 2015-2016 school year are confirmed, those programs will run on every school day during the school year. Schools that will offer before and after school programs in September 2015 will be confirmed in early spring 2015. Some schools will also operate on PA days and holidays to meet the needs of children and families. A listing of which schools will run on these extra days will be posted by the beginning of the 2015-2016 school year.
Please contact Tonya Millsap, Child Care Planner, at 705-734-6363 ext. 11297 if you have any questions.
Please note: the purpose of this survey is to determine whether the need for before and after school care in your school meets the minimum enrolment requirement; registration in a program is still required.  Your name and contact information is requested in the survey so the child care operator in your school can contact you to register. Only one survey response s required per family.
